Overview

The RiskXchange Questionnaire Designer is a flexible tool that allows users to create tailored assessments to evaluate third-party risk, compliance, governance, and other domains such as cybersecurity, ESG, and business continuity. Whether starting from scratch or using a template, the platform supports dynamic configuration, conditional logic, and seamless deployment.

Key Features of the Questionnaire Designer

- Sections
Group related questions for easier organization and improved clarity. Questions can be reorganized using drag-and-drop.

Choose from multiple question formats to capture appropriate data:
- Text
- Text Area
- Integer
- Multiple Choice
- Yes/No
- Maturity Choice
- Compliant Choice
- Free Text
- File Upload
- Date Selection
- Scored Responses
- Conditional Logic
Control when and how questions appear based on responses to earlier questions using the Link Question feature.

How to Use the Questionnaire Designer

Accessing the Designer

  1. Log in to your RiskXchange account.
  2. Navigate to the Questionnaires section.
  3. Click Create New Questionnaire.

Design Capabilities

- Add, remove, or rearrange questions and sections.
- Apply conditional logic to display questions dynamically.
- Mark questions as mandatory.
- Enable/disable options like Not Applicable, Additional Info, or File Upload.

Creating Questionnaires

From a Template

  1. Go to Create Questionnaire > From Template.
  2. Browse and select a template (e.g., ISO 27001, GDPR).
  3. Click Use Template.
  4. Customize by editing or removing questions.
  5. Save, assign, and publish the questionnaire.

Example: "Test Questionnaire From Template"
- Create a new questionnaire using "A Test Shared Questionnaire".
- Edit Q1 to say: "Tell us a bit more about what your company does."
- Move employee-related questions to a new section: Employees.
- Create section: Industry.
- Add & rename: "Select the industry that best represents what you do."
- Add Text Area question: "What other industries best represent your company?" with Not Applicable enabled.
- Create Compliance section.
- Add Compliant Choice: "Do you comply with all ISO standards?"
   - No file upload
   - No “Not Applicable”
   - Requires additional info if Not Compliant
- Add Maturity Choice: "How mature are your documentation and record-keeping practices?"
   - No additional info or file upload
   - Visible only if the answer to the above is Fully Compliant or Partially Compliant
- Save and publish.

From a Blank Canvas

  1. Choose Blank Canvas when creating a new questionnaire.
  2. Enter name and optional description.
  3. Add sections (e.g., Privacy, Legal, Cybersecurity).
  4. Add questions using various types and conditional logic.
  5. Preview and publish.

Example: "Test Custom Questionnaire"
- Create section: Employees.
- Add 3 questions:
1. Yes/No: "Do you have any employees?"
     - No file upload or additional info
2. Integer: "How many employees do you have?"
     - Shown only if Q1 is Yes
3. Multiple Choice: "Future Employees"
     - Description: "Tell us how many employees you are likely to hire over the next 12 months."
     - Options: None, 1-10, 10+
     - Requires additional info if None
     - Shown only if Q1 is No

Configuring Questions

- Optionality: Make responses required or optional.
- Not Applicable: Add this toggle where appropriate.
- Additional Info: Require justification for specific answers.
- File Upload: Attach supporting documents.
- Conditional Visibility: Show/hide questions based on responses to previous ones.

Tips for Effective Questionnaire Design

- Use descriptive section titles (e.g., Compliance, Employees, Industry).
- Keep questions clear and concise.
- Use conditional logic to keep the flow relevant and user-friendly.
- Test in Preview Mode before assigning.
- Reuse templates for consistency and efficiency.
